Adult Foster Care (AFC)

Are you looking for a family home setting for care? AFC is designed to provide supportive family living to elderly or disabled individuals who can no longer live alone but would prefer a family setting over an institution. AFC assists with activities of daily living (ADL), nursing oversight, and care management at home with 24-hour supervision. Both participants and caregivers must meet the criteria and guidelines set forth by MassHealth.

Eligibility

The eligibility requirements set out by the state are listed below. If you have any questions please feel free to contact an area office near your location.

To qualify for Adult Foster Care you must:
  • Be 16 years of age or older
  • Provide a summary confirming the appropriateness of
  • AFC services (to be given by your physician)
  • Require daily assistance with at least one Activity of Daily Living
  • (ADL), i.e, bathing, dressing, eating, continence care, ambulation or transfer
  • Be approved for clinical eligibility by the State
  • Be eligible for Medicaid (standard, SCO, or be able to pay privately)
  • Have a medical diagnosis and need daily personal care
  • Be able to communicate his/her needs (ok to be confused or forgetful)
  • Not be harmful to self or others
  • Be compliant with the plan of care

Standards Caregivers

We provide specialized assistance to individuals aged sixteen (16) years or older who are affected by a myriad of medical, physical, and or cognitive challenges.

What is Adult Foster Care (AFC)?

Adult Foster Care is a federally funded MassHealth program that is designed to provide living alternatives to individuals who are elderly or disabled. These individuals usually require assistance while performing activities of daily living (ADLs) and instrumental activities of daily living (IADLs).

ADLs include activities such as eating, toileting, dressing, bathing, transferring, and walking. IADLs are activities related to independent living that is incidental to an individual’s care, such as household management, laundry, shopping, housekeeping, meal preparation, transportation, and medication monitoring.

How does Adult Foster Care work?

As a licensed Adult Foster Care program, we provide industry-specific training to caregivers which assists them in delivering care in their homes.

We work with a team of expert registered nurses and Case Managers who carry out monthly home visits, monitor members’/clients’ health needs, and develop personalized care plans.
